Proper medical care is crucial to your health. At St. Paul's PACE in San Diego, we're committed to helping you receive the care you need so that you can remain living in your own home rather than in a skilled nursing facility.

As a St. Paul's PACE participant, you receive tailored medical care from a team of health professionals who specialize in aging and chronic conditions. Our doctors, nurse practitioners, and registered nurses work with therapists, dietitians, social workers, and home care nurses to build a plan that's right for you.

Specialists who work with your primary care physician.
A team of healthcare professionals works with you and your family to ensure coordinated care — getting to know you on an individual level. In addition to your primary care physician, we have specialists across a variety of fields.
All specialist appointments are handled through your PACE primary care physician and approved by your care team. St. Paul's PACE also offers virtual telehealth appointments to help minimize exposure for immunocompromised participants — and if you have a medical emergency outside regular business hours, our nurses are available 24/7.
Included with your Medical Care.
With St. Paul's PACE, all services are provided at no cost* to you if you have Medi-Cal or a combination of Medi-Cal and Medicare. If your health team deems you need care that Medicare and Medi-Cal do not cover, St. Paul's PACE may still cover it.
*Those with a Medi-Cal share of cost remain responsible for payment.
